Make sure to bring the completed form to the Tryout Check-In table before the tryout dates in 2024 and 2025 as specified by the TABU organization.

• 2. Player's First Name: Enter the player's first name.
• 3. School Attending: Enter the name of the school the player is attending.
• 4. Player's Street Address: Enter the player's street address.
• 5. City: Enter the city where the player resides.
• 6. Zip: Enter the postal code for the player's address.
• 7. Home Phone: Enter the player's home phone number.
• 8. Player's Cell (16U and older): Enter the player's cell phone number if 16U or older.
• 9. Player's Email (16U and older): Enter the player's email address if 16U or older.
• 10. Father's Name: Enter the father's name.
• 11. Father's Cell: Enter the father's cell phone number.
• 12. Father's Email: Enter the father's email address.
• 13. Mother's Name: Enter the mother's name.
• 14. Mother's Cell: Enter the mother's cell phone number.
• 15. Mother's Email: Enter the mother's email address.
• 16. Birthdate: Enter the player's birthdate.
• 17. Age Group Trying Out for: Select the age group the player is trying out for: 12U, 14U, 16U, or 18U.
• 18. Player Throws: Indicate if the player throws right or left.
• 19. Primary Position: Enter the player's primary position.
• 20. Bats: Indicate if the player bats right, left, switch, or slap.
• 21. Secondary Position: Enter the player's secondary position.
• 22. Other Positions: List any other positions the player can play.
• 23. Previous Travel Ball Experience and Training: List previous travel ball experience and training.
• 24. Specific Family Vacations or Other Sports: List any specific family vacations or other sports to be aware of.
• 25. Tryout Release: Player’s name, parent/guardian signature, and date of signing.

Failure to submit the tryout information sheet may result in the player not being allowed to participate in tryouts.
